Golf trivia: ten questions, every answer with its source

Where do the trivia answers come from?
Every question links its source: the National Library of Scotland for early golf, The R&A's own pages and Rules, the USGA's Rules of Handicapping, Scottish Golf History for the words, Quote Investigator for the quotes, the PGA Tour for strokes gained, TrackMan for the tour numbers, NASA for the Moon. If a fact is folklore it is not in the quiz.
Is the quiz the same for everyone?

Who hit a golf ball on the Moon?
Alan Shepard, on Apollo 14 in 1971, with a 6-iron head fitted to a sampling tool, one-handed in his suit. Two balls.
Why was golf banned in Scotland?
Because it distracted men from archery practice, which the kingdom's defence depended on. James II's Act of 1457 is the earliest written mention of the game; the ban was restated in 1471 and 1491.
